Reminder all athletes: Fill out your physical/alternate year forms!

If you are participating in a sport this fall, remember to fill out your health-related forms, which can be found here.

Sophomores and seniors can fill out the alternate year forms, while freshman and juniors should fill out the regular physical forms! Remember to also contact your doctor for their signature.

Have these forms ready for your coaches by the first practice, or else you will not be able to participate!
